FLY BOX VENTURES, LLC  DBA “QMAP LLC” is a software service that allows distribution company representatives to create profiles for purposes of providing them with access to QMAP tracking services for their equipment along with the Tray Check software used for Loaner Tray management. Users may change the portion of their profile information that they have supplied at any time in the QMAP and Tray Check systems; and, thus, they can control how other members and QMAP communicates with them. INFORMATION COLLECTION AND USE BY QMAP

QMAP collects information submitted by users such as name, email address, and mobile phone in order to provide communication methods within their respective organizations.

QMAP also logs non-personally-identifiable information, including, without limitation, IP address, profile information, aggregate user data and browser type, from members, other users and visitors to the QMAP web site. This data is used to manage the web site, track usage and improve web site services. This non-personally-identifiable information may be shared with third parties to provide more relevant services and advertisements to members. User IP addresses are recorded for security and monitoring purposes.

User profile information, including, without limitation, the pictures and names of members, is displayed in order to facilitate interaction between and among members on the QMAP web site. Email addresses and phone numbers are used for purposes of sending email and text notifications relating to the services provided by QMAP. Unless a member otherwise consents to having his or her email address shared or displayed, a member’s email address will not be shared or displayed to others. QMAP also may use a member’s email address to send updates, a newsletter or other news relating to the services provided by QMAP.

USE OF COOKIES

QMAP uses cookies to store the preferences and record the session information of visitors to its web site for purposes of ensuring that visitors are not repeatedly offered the same advertisements and customizing newsletter, advertising and web page content based on browser type and user profile information. We do not link the information we store in cookies to any personally identifiable information you submit while on our site. CORRECTING, UPDATING OR REMOVING INFORMATION

Members may modify any of their personal information at any time by logging into their account and accessing the Profile page.

THIRD PARTY ADVERTISING. Advertisements appearing on this web site may be delivered to members and other users by QMAP or one of its advertising partners. QMAP’s advertising partners may set cookies. These cookies allow the advertising partner’s server to recognize your computer each time it sends you an online advertisement. In this way, the server may compile information about where you, or others who are using your computer, saw their advertisements and determine which advertisements are clicked on. This information allows an advertising partner’s network to deliver targeted advertisements that they believe will be of most interest to you. This privacy statement covers the use of cookies by QMAP and does not cover the use of cookies by any advertising partners of QMAP.

SECURITY

QMAP member accounts are secured by member-created passwords. QMAP takes precautions to ensure that member account information is kept private. QMAP uses reasonable measures to protect member information that is stored within our database, and QMAP restricts access to member information to those of its employees and contractors who need access to perform their job functions, such as QMAP’s customer service personnel and technical staff. SHARING AND DISCLOSURE OF INFORMATION QMAP COLLECTS

Except as otherwise described in this privacy statement, QMAP will not disclose personal information to any third party unless we believe that disclosure is necessary: (1) to conform to legal requirements or to respond to a subpoena, search warrant or other legal process received by QMAP, whether or not a response is required by applicable law; (2) to enforce the QMAP terms of use or to otherwise protect QMAP’s rights; or (3) to protect the safety of members of the public and users of the services provided by QMAP. If QMAP believes it is necessary to disclose an individual’s personal information, QMAP will notify that individual of its intent to disclose the information when feasible. Notification will be made using information the individual provided to QMAP. QMAP reserves the right to transfer personal information to a successor in interest that acquires rights to that information as a result of the sale of QMAP or substantially all of its assets to that successor in interest. From time to time, QMAP may make changes to its privacy policy. If QMAP makes changes, it will post them on its web site to make users aware of what the changes are so users will always be aware of what information QMAP collects, how QMAP uses it, and when QMAP may disclose it. A user is bound by any changes to the policy when the user accesses or uses the web site after those changes have been posted. If, however, QMAP is going to use a users’ personally identifiable information in a manner materially different from that which is stated at the time of collection, QMAP will post notice of such change on its web site for a period of 15 days.
